About 12.30pm on Friday, the man was swimming with friends when he dived into the water and signalled for assistance, police said.
He was pulled unconscious from the water and emergency services were alerted to the incident.
Attempts to resuscitate the man lasted for more than an hour, as police and an Ambulance helicopter were sent to the scene.
Paramedics were winched down to a rocky ledge near the Southern Highlands swimming hole, with reports the man suffered a cardiac arrest.
He died at the scene.
Police continue to make inquiries into the incident, which they have labelled a “suspected drowning”, and will prepare a report for the coroner.
The incident is the 11th drowning in NSW since Christmas.
